Urology Treatment

Urology Treatment

Urology is the branch of medicine which deals with the function and disorders of the urinary system. Urinary system includes Kidney, Bladder and Uterus. Any disorder that affects the urinary tract often ends up affecting the reproductive tract. Urology includes the treatment of urinary disorders through both medication and surgical methods. Some of the disorders that can be treated with the help of medications and external therapies include prostatic hyperplasia and urinary tract infections. Drugs such as amoxicillin and ciprofloxacin are used to avoid urinary infection. The urinary surgery is mostly performed for the treatment of kidney stones, prostate cancer and stress incontinence, among other complex and serious disorders. A typical urology and genitourinary surgery can be performed with the help of open surgery and minimally invasive techniques such as laser and laparoscopic surgery.

Some of the urology procedures are:

Laparoscopic radical cystectomy is an advanced procedure for the treatment of muscle-invasive, organ-confined urothelial carcinoma of the bladder. The doctors will do some tests to ensure that the cancer has not spread and for the risk of comorbidities such as heart and lung disease. Before the laparoscopic radical cystoprostatectomy procedure, a general anaesthesia will be given. During the procedure, the surgeon will remove your bladder and prostate through several small incisions by the help of a laparoscope and thin instruments. A neobladder procedure will be done from the existing tissue. An ileal conduit might be required to complete the procedure. After the bladder is removed an ileal conduit is the new storage area for your urine. After laparoscopic radical cystoprostatectomy procedure, you will receive narcotics as required. You will be given intravenous (IV) fluids for nutrition until your digestive track recovers enough to take solid food. After the procedure, it will take approximately 6 weeks to recover.

Bladder neck incision is a surgical procedure performed to improve the flow of urine and to relieve urinary pressure in the bladder. The success rate of this procedure is about 60 percent. Once the procedure is completed, the symptoms of bladder neck obstruction typically subside. If an incision is unable to relieve the symptoms, then open surgery may be required. A general or spinal anaesthesia will be given to the patients before the bladder neck incision procedure. Then, the surgeon will insert a resectoscope through the urethra. After that, a cutting instrument will be attached to the resectoscope to make a small incision in the wall of the bladder neck. Finally, the obstruction is removed through the resectoscope.

You will need to stay for 2 to 3 days in the hospital. Lifting and straining should be avoided for 3 weeks as long as recovery is completed. Complications involved in a bladder neck incision procedure are pain, infection and bleeding.

It is a minimally invasive procedure used for the extraction of the bladder stone. This is an advanced method to avoid open cystolithotomy, which is more invasive and requires a larger recovery time. This procedure is usually performed under general anaesthesia. The surgeon will pass a small cystoscope through the urethra and into the urinary bladder. Then stone may be grabbed with a basket device and removed from the urethra. In case of a larger stone, the stone is fragmented into multiple smaller pieces using a laser.

An overnight hospital stay is needed in the hospital after the procedure, but you have to take oral antibiotics for 4 to 5 days. It will take more than a week for the complete recovery from laser cystolithotripsy will take. Complications of laser cystolithotripsy procedure are bladder pain, urinary tract infections, and ureteral injury.

Percutaneous nephrolithotomy (PCNL) is a surgical procedure performed for the treatment of larger kidney stones. This procedure is performed by a keyhole surgery through a 1 cm incision in your back. During the procedure, the surgeon removes the stone through a hollow tube inserted into your kidney and a probe through the tube. In this procedure, the stone is fragmented and then removed through the tube. A general anaesthesia will be given before the start of the procedure. PCNL is performed with the help of x-ray or ultrasound to guide the entry of the hollow tube into the kidney. Then, the stone will be fragmented and removed with the assistance of laser or stone grasping instruments.

Patients will be required to stay in the hospital for 2 to 4 days. Before discharge, the catheter will be removed. Around 6 weeks will be required to completely recover. Complications involved in PCNL procedure are excessive bleeding, infection and adjacent organ injury.

Radical nephrectomy is a surgical procedure performed to remove the kidney completely. This procedure is usually performed to treat kidney cancer or to remove a seriously damaged or diseased kidney.

There are two kinds of radical nephrectomy – open nephrectomy and laparoscopic nephrectomy. This procedure is generally performed through a single large incision in the abdomen or side known as open nephrectomy. In some cases, it is also performed through a series of small incisions in the abdomen known as laparoscopic nephrectomy. A general anaesthesia will be given before the radical nephrectomy procedure. During the procedure, the surgeon will make an incision on the side of the abdomen to reach the kidney. After the ureter and blood vessels are disconnected and the kidney is completely removed.

After radical nephrectomy, proper care is needed to preserve normal kidney function. You should eat a healthy diet, engage in daily physical activity and attend regular check-ups to monitor your kidney health.

The complications of radical nephrectomy are bleeding, infection and urinary leak. However, some more severe risks such as chronic kidney diseases and hypertension are also involved.
